How to install and activate Portable EHR on your device
1) Install Portable EHR
When your account has been created at a clinic, install the Portable EHR Patient application on your mobile device.
When you click on the app store icon below, you will be redirected to the "Apple™" app store, or to the "Google PlayStore™" depending on the make and model of your mobile device.
2) Activate Portable EHR
Once you have installed the application, you can open it and complete the activation with your patient information or by scanning a QR code that is being offered to you.
If you activate with your personal information, you will need the following information:
- The email address you used when you registered at the clinic
- The issuer of the identification card, for example RAMQ
- The identification number you used to register, for example your RAMQ insurance number
- The cellphone number you used when you registered at the clinic
Finally, to complete the activation, verify you email and cellphone number entering the PINs sent to you.
