Install Portable EHR on your device
When your account has been activated at a clinic, your first step should be to install the Portable EHR Patient application on your mobile device.
When you click on the app store icon below, you will be redirected to the "Apple™" app store, or to the "Google PlayStore™" depending on the make and model of your mobile device.
Once you have downloaded the application, you can open it and complete the activation with your patient information or by scanning a QR code that is being offered to you.
If you activate with your personal information, you will need the following information
- The email address you used when you registered at the clinic
- The identification number you used to register, for example your RAMQ insurance number
- The issuer of the identification card , for example RAMQ
- The clinic at which you registered.
